- Understanding and learning about your medical condition
- Schedule appointments with you doctor
- Attend doctor visits with you to help answer questions
- Assist to arrange transportation to doctor appointments
- Help you get your medicine and any equipment you may need
- Visit in your home for teaching about your medical condition.
