How Can I Learn More about Managing My Symptoms of Chronic Disease like Diabetes, Asthma, Congestive Heart Failure or COPD?

If you are a Carolina Access Medicaid client and under the care of a participating physician, you are automatically eligible for services provided by Debbie Morris and Tracy Perry,  Registered Nurse Case Managers for Carolina Access Medicaid. Their office is located at Carteret County Health Department and can be reached at 252-222-7784 (Debbie) and 252-222-7741 (Tracy).  They can assist you with:

• Understanding and learning about your medical condition
• Schedule appointments with you doctor
• Attend doctor visits with you to help answer questions
• Assist to arrange transportation to doctor appointments
• Help you get your medicine and any equipment you may need
• Visit in your home for teaching about your medical condition.
