What is the Child Service Coordination Program?
This is a free and voluntary program to assist eligible families find and use community services. We can help you find medical care, transportation, childcare and financial aid.
What are the Goals of the Program?
- To connect your family with services.
- To support your children in reaching their developmental potential.
- To help ensure that children are raised in healthy, safe and nurturing homes.
How can the Child Service Coordination Program Support My Child and I?
- Help determine your family’s strengths and concerns through regular home visits and telephone calls.
- Using the “Ages and Stages Questionnaire” to monitor your child’s development.
- Suggest age-appropriate activities to help your child learn.
- Answer your questions about behavior, discipline and positive social-emotional development.
- If needed, referrals to the Infant-Toddler Program and the Child Development Services Agency (CDSA) for additional services.
Who is Eligible for the Child Service Coordination Program?
- Families with children birth to three who are at risk for developmental delay, developmental disability or chronic illness. Also, children three to five years of age with specific special needs are eligible.
- There are no financial criteria for the program. Families of all income-levels can receive services.
How Can My Family Get a Child Service Coordinator?
Families may request services directly or be referred by a variety of agencies and individuals like family members, hospitals, and childcare centers.
